Accompanying Materials

This is the term given to materials which are part of the thesis but which are not included in the text (e.g. CD, DVD, CD-ROM).

Submitting CDs and DVDs

When the thesis consists only of an audio or video recording, such as a musical recital, film or play, you must submit the following to the College of Graduate Studies:

  • one high-quality copy of the CD or DVD
  • paper copy of the title page
  • signed paper copy of the Dissertation or Thesis Approval form with original signatures
  • signed paper copies of all applicable forms
  • paper copy of the abstract
  • one copy of the performance programme (for audio or video performances)

Labelling CDs and DVDs

Each item submitted must be in its own case and must be labelled as follows:

On the case: a 1” X 3” label containing the following information:

  • student's last name, first name
  • degree to be awarded (e.g. MA, PhD)
  • date of graduation ceremony (e.g. Spring 2009, Fall 2009)
  • label each set of materials with the copy number and number of copies e.g. 1 of 2, 2 of 2. Example: if your accompanying materials are contained on two CDs, they must be labelled 1 of 2 and 2 of 2
  • thesis title

On discs in permanent marker:

  • student's last name, first name
  • degree to be awarded (e.g. MA, PhD)
  • date of graduation ceremony (e.g. Spring 2009, Fall 2009)
  • label each set of materials with the copy number and number of copies e.g. 1 of 2, 2 of 2. Example: if your accompanying materials are contained on two CDs, they must be labelled 1 of 2 and 2 of 2
  • thesis title

Please do not stick any labels directly onto the discs. We will not be able to accept them.
