This form should be used when nominating a student for the UBC Okanagan Thesis Award. This form must only be submitted by a student's primary supervisor.

Supervisor Nomination*
Max. file size: 12 MB.
A one page letter composed by the student's Supervisor in support of the nomination, including a clear articulation of how the thesis is of exceptional quality and originality, and makes a significant contribution to their academic field.
Abstract*
Max. file size: 12 MB.
An abstract of the thesis not exceeding 350 words, written by the student in non-technical language.
Student's CV*
Max. file size: 12 MB.
The student's CV should include scholarships and awards, contributions to research and publications, work, etc.
DECLARATION AND SIGNATURE
I declare that the statements made in this submission are, to the best of my knowledge, true, complete and correct. I understand that the Dean of the College of Graduate Studies at the University of British Columbia's Okanagan campus reserves the right to revoke any scholarship offering resulting, where necessary, in the need for a student to withdraw from his/her degree program if any of the information in this reference is found to be untrue, or if evidence of fraudulent submission of documentation is found.
